When it comes to managing diabetes, you are the most important member of your health care team! Diabetes can be complex and managing it can sometimes feel overwhelming. To make it a little easier, the American Association of Diabetes Educators (AADE) has outlined the AADE 7 Self-Care Behaviors™ for diabetes self-management. Good news – you don’t have to do it alone. The Brook Experts are here to help!

1. Healthy Eating
Here at Brook we’ve developed the Brook Healthy Plate as a guide to creating blood sugar friendly meals. Our Experts can support you with important healthy eating tips such as increasing fiber intake and creating balanced snacks, just to name a few. Best of all, you can feel confident knowing that a team of health professionals are keeping an eye on the foods and blood sugars you report. The Brook Experts provide feedback, look for connections, and notice things in your data that you might not. They love talking about food and are available to chat and answer questions about all aspects of healthy eating.
2. Being Active
Brook can sync to your phone or fitness wearable to help keep track of your steps and exercise. The Brook Experts can help you set goals, provide accountability, and show how activity affects your blood sugar readings.
Using the Brook app, you can record your blood sugar readings and some glucometers can sync that data directly. You’ll be able to see trends in your blood sugar over time, or related to activity and food. The Brook Experts are there to provide feedback on how to manage highs and lows, and suggest adjustments in your routine to improve those readings.
4. Taking Medication
You can report your medication schedule and track your medication administration in the Brook app. It’s important to remember to take your medications as prescribed, and Brook is here to help you remind you! The Brook Experts can answer questions about medications such as managing possible side-effects and when to chat with your doctor regarding medication concerns.
5. Problem Solving
Keeping track of your blood sugar readings, food, and activity makes it easier to see patterns and how all of those pieces affect each other. The Brook Experts are also available to help you troubleshoot, remove barriers, and decide what changes to make to keep your blood sugar in the safe zone.
6. Reducing Risks
Using Brook and chatting with the Brook Experts encourages you to adopt healthy behaviors that not only helps manage diabetes but can also reduce risk of diabetes-related complications and other health concerns.
7. Healthy Coping
Brook’s Experts are another addition to your network of support for managing diabetes. They are available to chat, right in the app, 7 days a week. They have ideas for stress management and healthy coping strategies. The Brook Experts are here to help!
